The tool assesses if a breach poses a risk of significant harm and whether the breach must be reported, the OPC said. Once the questionnaire is completed, the tool will indicate whether significant harm is likely or unlikely, enabling the organization to decide what action to take.
The tool takes users through a series of questions that analyze key details of the breach, the OPC said. It doesn't ask for information that identifies the organization, and the information provided isn't sent to the OPC. The tool is just "one element to consider in assessing a breach" and its "results do not replace your own judgment."
